New Report Confirms Benefits of Platelet-Rich Plasma in Hair Loss Treatment

Platelet-Rich Plasma in Hair Loss TreatmentEvery year, doctors and researchers make advances in the treatment of hair loss. We have come a long way in the last decade or so in developing more effective and less invasive procedures and techniques that result in more natural-looking heads of hair. Remember plugs? We barely do, as technology has long since passed them by. Now, new research has added further support for a potentially revolutionary treatment for certain types of pattern baldness which uses the body’s natural healing processes to restore lost hair.

Platelet-rich plasma (PRP) is a new biotechnology which has been used with increasing frequency to aid in the healing of injuries or speed recovery after surgery and is also being used to help fix dermatological problems.

Now, a report just published in the journal Skin Appendage Disorders suggests that PRP may help in the treatment of androgenic alopecia, the most common form of pattern baldness. Over 95 percent of hair loss cases in men and women is caused by androgenic alopecia, and up to 80% of men and 50% of women are affected by androgenic alopecia during their lifetime. 

What is Platelet-Rich Plasma?

Platelets are the clotting cells of our blood, but they are also enriched with growth and healing factors that will initiate repair and provide assistance to stem cells in damaged muscle, tendon, and ligaments.

PRP contains various growth factors and cytokines that enhance the body’s inherent capacity to repair and regenerate. PRP therapy essentially takes the body’s natural processes and essentially turbocharges them. A sample of blood is taken from the patient and then placed in a centrifuge where the platelets are separated from other elements of the blood. As used in the treatment of injuries and trauma, that platelet-rich plasma is then injected into the injured tissue, stimulating the work that the body is already doing. 

How Does PRP Treat Hair Loss?

After reviewing 12 previous studies involving PRP and the treatment of androgenic alopecia, 10 of which “remarked positively on the therapeutic potential of PRP for the treatment” of the condition, the authors concluded that “the available evidence suggests a promising use for PRP as an alternative treatment for androgenic alopecia.” 

The report noted that PRP contains high concentrations of over 20 growth factors, including numerous factors, thought to stimulate hair regrowth. The authors found that those growth factors in PRP “promote hair regrowth by binding to their respective receptors expressed by stem cells of the hair follicle bulge region and associated tissues. Upon ligand binding, stem cells induce the proliferative phase of the hair follicle, producing the anagen follicular unit and facilitating hair regrowth.”

What that means in English is that PRP appears to nurture hair follicles with critical growth factors that take them from a dormant state (in which hair is no longer growing) to an active state (in which a normal hair growth cycle occurs).

Dr. Rose: You gave a great talk. I thought it was a great lecture, and it points to the fact that we often miss how to take care of patients. The fact that you had that survey that talked about what patients perceive. And I will tell you– and I’m not blowing smoke here. When I had gotten back from that meeting, and I had written down some of the things that you had said, some of the comments that patients had about how they were treated. I immediately got back and said to my staff, [and] asked them to put in to print the questions that patients felt they were not being asked. And if we did ask those questions, to make sure that not only the people who consult for me, but we as physicians, Dr. Nusbaum and I, speak to that patient particularly about those issues. In particular, I think you had mentioned asking the question, ‘How is your hair loss affecting you emotionally, in your social situation, in your lifestyle?’ And I think for the most part, patients are very welcoming to get that question and have an opportunity not so much to vent, but to just say, ‘This is what’s bothering me about my hair.’ There are some people who don’t really want to talk about it, we learned. But there were questions you brought up that we immediately put into place.

Spencer Kobren: Well, I appreciate that. And my whole thing is really about creating the best possible patient experience. When patients come to me, and they say, ‘You know what, it doesn’t even seem like the doctor cared. All they wanted to do was make that sale, get me in the chair, and get me out.’ And when I hear [about] guys, even from well known practices, who aren’t following up personally. Unless you’re doing 30 cases a day, and I don’t think that’s happening in most practices, it’s so easy to make a 2 minute phone call and say, ‘Hey, how are you doing? I just wanted to make sure everything is okay. I’m going to have so-and-so from my staff check-in on you tomorrow.’ All of the sudden, they realize, this doctor really cares. And I know you guys do that in your practice.

Dr. Rose: We try. Every patient has my cell phone number. We try to be very forthright with patients. If I think they are a good candidate, I tell them. If I don’t think they’re a good candidate, I’ll tell them, ‘Have you ever looked at yourself with your head shaved? You might be a Michael Jordan, you might be a Sean Connery. It could work really well for you.’ There’s a lot of stuff out there now about PRP, patients asking us about that. And there’s a lot of– to me– pseudo science related to some of these things. I try to be very careful about what I say to patients and what I think is accurate science about some of these things, which we do use. But, in any case, I thought that your lecture was very beneficial for all of us at the meeting. I did have some takeaways there that I put into place right away.

What is PRP?

As the name implies, PRP is simply a mixture of blood plasma that has additional platelets, growth factors, and cytokines mixed in. PRP is always prepared from the patient’s own blood sample. The volume of natural platelets, growth factors, and cytokines makes platelet-rich-plasma an ideal addition to any procedure that might require an enhanced healing process.

What is PRP used for?

Some individuals might experience minor shedding or hair loss following hair transplant surgery. Fortunately, most men and women who lose hair after transplant are able to grow it back naturally.

So why do some people lose hair after a restoration procedure, and are there ways to limit your chance of developing such shock loss?

What is Shock Loss

Shock loss is a term some hair loss professionals may use to describe a specific type of hair loss that occurs after the transplant. For many patients, shock loss is only temporary. The risk of shock loss varies by patient and is always described in detail during hair transplant consultation.

In general, shock loss is caused by the trauma undergone during transplant. The more trauma induced, the greater the risk of shedding or loss after the procedure. By the same token, the risk of shock loss can be greatly reduced when surgeons use certain techniques and technologies prior to surgery.

This new method for hair restoration has demonstrated the potential to stimulate cellular activity deep with hair follicles, flooding them with the blood, oxygen, nutrients, platelets, and other natural growth factors needed to reduce inflammation and maximize the effectiveness of transplant procedures.

Treating Hair Loss with PRP

Treating hair loss with PRP begins with drawing a small amount of the patient’s own blood at the time of the procedure. Next, the natural healing and growth factors of the blood are enhanced through a special process that concentrates the platelets. This concentrated solution is then administered to both donor area and recipient area during the procedure.

When applied to the donor area, PRP seems to ward off inflammation, helping the area to heal more quickly and efficiently. There is some evidence that suggests PRP helps to reduce the appearance of post-operative scars (though some visibility is still apparent). Scientific Study: PRP Significantly Increases Hair Growth

According to evidence published in the British Journal of Dermatology, “PRP was found to increase hair growth significantly” in patients suffering with alopecia areata (i). These results were achieved through a randomized, double-blind study that compared patients treated with PRP with those treated with a placebo. Researchers also noted significant decreases in hair dystrophy (fragility), burning, and itching among patients who were given PRP treatment.

Hair Loss Treatment 2013: What’s New at the Miami Hair Institute

Both Dr. Rose and Dr. Nusbaum have over 25 years of experience in the field of hair restoration providing a solid foundation and track record for evaluating the best new techniques available to you. We are very excited to tell you about some new developments which we have incorporated into our practice.

Robotic Hair Restoration

In addition to the strip harvesting technique, we also offer a donor harvesting method named FIT Follicular Isolation Technique (also known as FUE). Using this technique the creation of a linear donor scar is eliminated. While this procedure has gained recent popularity, Dr. Rose has been performing it for over ten years with a special, hand-held motorized instrument and we continue to offer the hand motorized FUE . We also offer the ARTAS physician guided robotic system for FUE, FIT which is the most technologically advanced instrumentation worldwide. The ARTAS robotic instrument utilizes advanced digital technology and laser guided precision robotics to harvest permanent hair from the back of the head to be transplanted into balding or thinning areas. This is a minimally invasive procedure, with no linear scarring and less postoperative pain.  ARTAS produces tiny dot- like scars in the donor area that are not visible with a short haircut although complete shaving of the head would reveal them. We believe the best candidates for this procedure are individuals who have never had a strip harvest procedure and wish to have transplants done yet want  be able to wear their hair quite short  with minimal evidence  of a procedure .

Biolife

Donor follicles, once harvested, must be maintained in a viable state until they can be re-implanted in the recipient area; and the entire process must take place within hours. The standard liquid solution, which has been used for years to hold follicular grafts during surgery, is saline water. Although this has been effective, we are now utilizing a product called Biolife, which is equivalent to the solutions that are used to preserve organs for transplantation.  Preserving your follicles in Biolife further ensures that they will remain viable during the procedure. In our experience, it has increased follicle growth while at the same time, producing earlier growth than what we were getting with standard methods. Biolife contains nutrients that mimic the natural environment of the follicle cells within the tissues therefore minimizing the stress that the follicles must endure during the transplant process.

Platelet Rich Plasma (PRP)

PRP is prepared by drawing a small amount of your own blood at the time of the procedure. We then utilize a special method to concentrate your platelets, the latter which contain numerous growth and healing factors. PRP is injected into both the donor and recipient areas once they are already anesthetized and accelerates healing as evidenced by less redness and crusting in these areas.  PRP may also improve the percentage of growth of transplanted follicles. We recommend PRP for all procedures but particularly for patients who have had previous transplants. In our experience patients who use PRP develop much less inflammation at the surgical sites, heal quicker and, although we have not scientifically studied this, may have improved hair growth.

EXPAREL

Although the vast majority of our patients experience very little discomfort, requiring pain medication only during the first 24 hours post-op, we are always looking to improve the patient experience. Exparel is a new long-lasting local anesthetic that is contained in liposomes which act as a timed-release system.

To reduce your discomfort after the procedure, Exparel is injected into your donor area during the surgery and is specifically designed to release the anesthetic gradually, controlling postoperative pain for up to 48 hours.

While their constant focus is on patient care, Drs. Nusbaum and Rose continue their ongoing commitment to research and teaching. Recent medical publications include  “Nonsurgical Therapy for Hair Loss ” in the Facial Plastic Surgery Clinics of North America,  “Repair of the Height of the Hairline after a Juri Flap” in Hair Transplant  360 and “Surgical Considerations for Patients with Frontal Fibrosing Alopecia” in the Hair Transplant Forum International. They also created an instructional video on hair transplantation for plastic surgeons, which was published in “Hyperguide,” a plastic surgery online journal.

Both Doctors Rose and Nusbaum will be lecturing at the International Society of Hair Restoration meeting to be held in San Francisco later this month. This is the largest hair restoration conference, bringing together over 700 physicians from around the world.

Scientists Have Success with ‘Vampire Treatments’ for Baldness

Scientists Have Success with ‘Vampire Treatments’ for BaldnessResearchers have announced success in re-growing patients’ hair with a special solution of their own blood. These so-called vampire treatments for baldness involve strategically injecting areas of balding scalp with solutions of platelet rich plasma, or PRP, derived entirely from the patients’ blood.

The research is the product of scientists from the International Hair Research Foundation and University of Brescia, Italy. Just fewer than 50 patients were observed, and while many received PRP injections, a control group received only a placebo. Three total injections were administered to each patient, after which hair growth was measured. Researchers observed significant hair re-growth in patients treated with PRP injections, showing a strong correlation between PRP therapy and hair growth. Follow-up studies are to be conducted by teams at the Hebrew University Medical Centre in Israel, according to sources familiar with the matter.

“Alopecia is known to lead to overwhelming effects on the patient’s quality of life and self-esteem,” says Nina Goad of the British Association of Dermatologists, commenting on the importance of the research findings. “This could offer hope to thousands,” (i).

And while each patient in the study was treated for alopecia areata, researchers believe the injections may also help individuals suffering with pattern baldness.

“We think it can help to regrow hair on people with androgenic alopecia (male-pattern baldness),” explains Dr. Fabio Rinaldi. “We believe it is the best treatment available, apart from surgery.”

Use of PRP in Hair Transplant Surgery
